Student Health Care Plans
On enrolment you will be asked to provide your child’s health information to help the school meet your child’s health needs.
You may need a Health Care Plan if they require health care support at school.
Medical Action Plans
The attached medical forms for common medical conditions are attached below.
These forms must be signed by a doctor before they are returned to the school.
Medication
If your child needs to be given medication during school hours, you will need to provide:
- Medication that is labelled with your child’s name, in its original packaging and within expiry date.
- Written authorisation for the school to administer the medication using a standard form from the school.
This applies to medication for long-term and short-term use.
